How To Get Help

Now that you have had a chance to understand more in detail about the Angina Management Programme; who it is for, how we work and what the Angina Management Programmme can do for you we would like to give you the next steps in order to get help.

As long as you qualify, and are interested in what the Angina Management Programme can do for you, it is time to consult with your clinician to get a referral:

  • If you are currently attending a consultant clinic at hospital, your consultant will be able to refer you.
  • If you have been discharged from hospital, your GP will be able to refer you.
  • If you don’t live in Derbyshire but are interested, we can sometimes arrange with your local NHS to fund the service for you. Please complete the form below
